To be eligible to
participate in the levelized payment plan, you must have completed
at least eleven months prior service at the address for which you
wish to use the levelized payment plan.
1. The applicant
whose signature appears below requests that charges for natural gas
service be billed in levelized billings, and agrees to pay each
installment on or before the due date printed on each monthly
bill.
2. The amount of each
monthly installment will be calculated by adding actual usage from
the previous eleven months to actual usage for the current month.
The total is then divided by twelve to derive an average usage. The
average usage is then multiplied by the current Btu factor to
derive the therms to be billed. The levelized payment plan amount
due is then calculated by applying the current rate schedule to the
therms to be billed, and rounding to the nearest whole dollar. This
calculation is repeated each month.
3. Failure to pay any
monthly levelized payment plan installment when due will
automatically cancel this agreement unless satisfactory
arrangements for future payments have been made.
4. This agreement
applies to the above address and applicant only. If service is
terminated at any time, this agreement will immediately terminate
and any amounts payable by the applicant will be paid on the final
bill. Any overpayment will be refunded by check.
